Robotic Knee Replacement: Where Technology Meets Surgical Experience

Knee pain is often something people gradually learn to live with. Initially, climbing stairs or walking longer distances may become slightly difficult. Over time, everyday activities such as getting up from a chair, going to the market or even taking a short walk can become painful.

For patients with advanced knee arthritis who continue to experience significant pain despite appropriate non-surgical treatment, knee replacement can provide an opportunity to regain mobility and return to everyday activities.

Over the last few years, robotic-assisted knee replacement has introduced an additional level of technology to this commonly performed procedure. However, there is often confusion about what robotic knee replacement actually involves and whether it is necessarily better for every patient.

The important point is that the robot does not replace the surgeon. Instead, it is a technology that assists the surgeon in planning and performing the operation with greater precision.

What Happens During Robotic Knee Replacement?

Knee replacement traditionally involves carefully assessing the patient’s deformity, planning the bone cuts and positioning the implants to restore appropriate alignment and balance.

With robotic assistance, this process can become more personalised.

The patient’s knee anatomy is assessed before or during surgery, depending on the robotic system being used. A computer-generated plan helps the surgeon understand the patient’s individual anatomy and determine the appropriate positioning of the implant.

During surgery, the system provides real-time information about alignment, bone preparation and knee balance. The surgeon can use this information to make adjustments and achieve the planned correction.

The final decisions, however, remain with the surgeon.

Why Does Precision Matter?

A knee replacement is not simply about removing the damaged portion of the joint and replacing it with an implant.

The knee has a complex relationship between the bones, ligaments and surrounding soft tissues. Restoring alignment and achieving appropriate soft-tissue balance are important components of the procedure.

This becomes particularly relevant in patients with significant deformity, previous surgery or unusual anatomy.

Robotic assistance can provide the surgeon with additional information during the procedure and help execute the planned bone cuts and implant positioning accurately.

It is this combination of individualised planning and intraoperative information that makes robotic technology an important development in modern knee replacement.

Is Robotic Surgery Better Than Conventional Knee Replacement?

This is one of the most common questions patients ask.

There is no single answer that applies to everyone.

Robotic systems can improve the precision and reproducibility of certain aspects of knee replacement. However, technology alone does not determine the outcome.

The patient’s condition, severity of arthritis, type of deformity, ligament stability, bone quality, implant selection, surgical technique and postoperative rehabilitation can all contribute to the final result.

A well-planned conventional knee replacement performed by an experienced surgeon can also provide excellent results.

Therefore, robotic technology should be considered an additional tool in the surgeon’s hands rather than a replacement for surgical experience.

Who May Benefit From Robotic-Assisted Knee Replacement?

Robotic assistance may be particularly useful when the anatomy is complex or when precise correction is required.

Patients with severe deformities, previous operations around the knee, unusual bone anatomy or ligament imbalance may require more detailed planning.

At the same time, not every patient who needs a knee replacement necessarily needs robotic assistance. The decision should be made after a detailed clinical examination and review of appropriate imaging.

The right technology is the one that is appropriate for the individual patient.

What About Recovery After Robotic Knee Replacement?

Patients are often surprised when they hear that the recovery process does not depend entirely on whether the surgery was performed using a robot.

Early mobilisation, pain management and physiotherapy remain important after knee replacement.

Most patients are encouraged to start moving the knee and walking with appropriate support soon after surgery. Rehabilitation then focuses on regaining movement, reducing swelling and rebuilding strength.

A patient’s general fitness, muscle strength before surgery, age, weight, severity of arthritis and commitment to rehabilitation can all influence recovery.

In other words, the operation is only one part of the journey.

The Technology Is Useful, but the Surgeon Remains Central

One of the biggest misconceptions surrounding robotic surgery is that the robot independently performs the operation.

That is not how robotic knee replacement works.

The surgeon plans the procedure, interprets the information provided by the system and performs the operation. If the findings during surgery differ from the initial plan, the surgeon can make appropriate changes.

This is particularly important in complex cases, where experience and clinical judgement remain essential.

In my practice, I see robotic technology as an extension of surgical planning and precision rather than as a substitute for the fundamentals of good orthopaedic surgery.

Looking at the Bigger Picture

The purpose of knee replacement is not to use the latest technology simply because it is available.

The purpose is to help a patient walk with less pain, improve function and return to a more active and independent life.

Robotic-assisted knee replacement can contribute to this goal by allowing more personalised planning and providing the surgeon with detailed information during surgery. However, good outcomes still depend on choosing the right patient, selecting the appropriate implant and surgical technique, performing the operation carefully and providing proper rehabilitation afterwards.

For anyone considering knee replacement, it is important to discuss the complete treatment plan with an orthopaedic surgeon rather than focusing only on whether the surgery is “robotic” or “non-robotic.”

Technology can improve what we do.

But it is the combination of technology, surgical experience, sound clinical judgement and patient-specific care that ultimately matters.
